• Confidential Communications – Request that we communicate with you in a certain way (e.g., at a different address or phone number).
• Restrictions – Request limits on how we use or disclose your PHI (though we may not always be able to agree).
• Accounting of Disclosures – Request a list of disclosures we have made of your PHI, other than those for treatment, payment, or operations.
• Paper Copy – Receive a paper copy of this privacy notice at any time, even if you have agreed to receive it electronically.
Safeguards and Security
We use appropriate administrative, technical, and physical safeguards to protect your PHI against unauthorized access, disclosure, alteration, or destruction.
Changes to This Policy
We reserve the right to change this Privacy Policy at any time. Any revised policy will apply to all PHI we maintain and will be available in our office and on request.
